WHAT TO EXPECT

INITIAL VISIT

The purpose of this visit is to perform a comprehensive evaluation of the patient. This will include detailed history taking from birth until present; as well it may include testing with the patient. You will be seen by the physician and support staff during this visit. If the assessment is completed within this appointment time you will receive results on the same day.

LIST OF ITEMS TO PREPARE
  • You must present a VALID ONTARIO HEALTH CARD at every visit. Failure to do so could result in interruption of services.
  • Please fully complete our intake questionnaires, and submit online or return prior to your first visit. These play a vital role in the assessment process, and if incomplete delays in assessment could occur.
  • Please allow 1.5-2 hours for you first appointment.
  • Please arrive 15 minutes prior to your appointment for registration.
  • Please bring any prior assessment reports, school report cards, Individual Education Plan or any other documentation that may be relevant to our assessment.”
  • If there are any child custody arrangements, relevant documentation (ie. court orders) is required for our records prior to accessing services. In addition please complete the Parental Agreement Form and return at your first visit.
SUBSEQUENT VISITS

The purpose of these visits is to review the current status of the patient. We will monitor symptom impairment, any new difficulties or areas of need, and treatment efficacy whether pharmacological or other interventions. Patient must attend these appointments. Please allow 30-45 minutes for this visit.
